Hello! Every student planning to participate in extracurricular activities must have a signed Activity From (just like in past years). What's different this year, is that middle schools have their forms on a new Leon Schools platform called Aktivate.com.
How to use Akitivate.com to fill out your Field Trip Permission / Activity Form
- Parents must create an account and add their student as an "athlete" (even if they're not playing a sport.)
- Once they are added as an athlete, they must start a new registration for their student (there's a blue button that says "New Registration +") and from the list of sports, choose the sport called "Field Trip."
- Once selected, the system will take you step-by-step through the Activity Form process, including e-signing the form and submitting it to the school.
