    • 2 proofs of residency
    • Certified copy of birth certificate
    • physical (within a year-after 8-15-16)
    • Proof of immunization
    • Social Security Number (requested)

    Bring your child to meet their teacher.  Classrooms will not be open until 1:30 pm
