NEED TO MAKE CHANGES TO YOUR BENEFITS DUE TO MARRIAGE, DIVORCE, BIRTH, ADOPTION, DEATH, OR LOSS OF COVERAGE?
You only have 30 calendar days from the date of the event to make these changes. Day 1 of the 30 days is the date of the event. Contact the Benefits Department via email for more information.
If you are a new employee and eligible for benefits, you have 30 calendar days to enroll.
Day 1 of the 30 days is your date of hire. Contact Deborah Hunt with RGVI/HUB at 850-597-0401.

The user name is your SS# and PIN is last 4 of your SS# and last 2 of your birth year.

IMPORTANT HEALTH INSURANCE INFORMATION
The Capital Health Plan Value Selection Plan and Florida Blue 05172/05173 Plans have a high deductible. The deductible is per calendar year which is January 1st - December 31st. The Out of Pocket Maximum is also per calendar year.
