Voluntary Pre K (2nd appointment for VPK-eligible children)

  • Your journey with Early Childhood Programs starts with an appointment for a 30-45 minute screening for your child.  Your child must be on or past his third birthday for you to make an appointment.
    What to bring with you to a VPK enrollment appointment
    1. VPK certificate

    2. Florida Certification of Immunization, DH Form 680
      If form has a Part B Temporary exemption, it must include a valid expiration date.

    3. School Entry Health Exam form (Form DH3040-CHP-07-2013), a record of a physical examination completed within the 12 months preceding the first day of school enrollment and attendance.  The school exam form is found here at
