Medication on Campus
If your student needs to take medication during the school day, please complete the required forms found in the Clinic Documents section under Attendance on the Leon High School website.
A separate form must be completed for each medication and submitted to the school clinic along with the medication.
Important Guidelines

All medications must be brought to school by a parent or guardian.

Students are not permitted to carry medications on campus unless it is medically necessary for asthma, diabetes, seizures, or severe allergies.

If a student needs to carry medication, a Permission to Carry form must be on file with the clinic.

Any student found carrying medication without proper documentation will have the medication confiscated and will be referred to school administration.

Field Trip Medication Administration
If your student requires medication during an extended day or overnight field trip, a Medication for Field Trip form must be completed for each medication.
If the medication is for asthma, diabetes, seizures, or severe allergies, a Permission to Carry form is also required.
Field Trip Guidelines

Students may not carry any medications—including over-the-counter medications (e.g., Tylenol) or prescriptions (e.g., birth control)—unless covered by a Permission to Carry form.

All medications must be delivered by a parent or guardian to the designated trip supervisor.

Medications must be in their original packaging or prescription bottle.

If a student is found carrying medication without the required documentation:

The medication will be confiscated.

Parents will be contacted.

The student may be sent home from the trip at the parent’s expense.
