    Student Health Cards are required each year for all Leon County Students.

    The 2020-2021 School Year Health Forms are located on-line in the Parent Portal.  When you sign into the Parent Portal you will be prompted to complete a Health Card for each child and WILL NOT be permitted to access grades, etc. until the card is completed. Please check and update each year.

     

    Medication on Campus:

    If your student requires medicaiton on campus please complete the forms located in the Attendance section of the Leon High School website, under Clinic Documents.  Print a seperate copy for each medication and submit it, along with the medication, to the Clinic.  Please read Accepting Medication from Parents for more information. ALL MEDICATION MUST BE BROUGHT IN BY A PARENT.  STUDENTS ARE NOT PERMITTED TO CARRY MEDICATION ON CAMPUS UNLESS MEDICALLY NECESSARY FOR SEIZURE, ASTHMA OR DIABETES. Any medicaiton carried by a student must  have a Permission to Carry form on file with the Clinic.  Any student found to be carrying ANY medication without a permit will have the medication confiscated and they will be sent to Administration.  Additionally, Health Plans are located in the Clinic Documents section of this website.

     

    Field Trip Medicaiton Administration:

    If your child will require medication during an extended day or overnight field trip, please complete a  Medication for Field Trip form for EACH medication and, if the medicaiton is for Seizure, Asthma, Severe Allergy or Diabetes, please complete a Permission to Carry form.  NO OTHER MEDICATIONS WILL BE PERMITTED FOR CARRY!!!  Even over the counter medicaiton, such as Tylenol, or birth control/hormonal medication will require a form and the student may not bring it to school.  It MUST BE transported by the parent to the appropriate person designated for the field trip.  All medicaiton must be in their origianl packaging or perscription bottle.  Medication found on a student without the appropriate forms will be confiscated, the parents notified, and it may result in the student being sent home from the field trip at the parents expense.
