    Leon County Schools has improved the school athletics registration & documentation process by partnering with Aktivate, Inc. This new platform will make the registration & eligibility process cleaner and easier for the student-athlete, parent and campus staff.

    What is Aktivate?

    Activate is a digital athletics management platform that brings all athletics registration, documentation and management into a single, “one stop” platform space. This will eliminate the need to deliver paper copies to campus by families and make all aspects of athletic file management easier for schools. Aktivate has a high level of customer service to provide you support throughout your registration.              

    What do I need to have in order to begin registering my student?

    Families will need:

    1. Their student’s insurance coverage information.
    2. Their student’s Leon County Schools issued Student ID number (this can be found in their FOCUS portal).                                                      

    What forms are required to be submitted for my child to be eligible?

    1. A current, doctor signed EL2-Physical Evaluation.
    2. An Activity Participation form (can be e-signed).
    3. A Student Health Disclosure (HIPPA) form (can be e-signed).                                                                                                                                 

    Where do I get these forms?

    Each form will populate electronically for download or e-signature in the registration process.           

                                                                      

    I have turned in documents that I believe are still current, will I need to resubmit them?

    Yes, the Activity Participation & Student Health Information Disclosure (HIPPA) forms are good for one academic year only. They must be renewed before the start of the new school year.  The current forms will populate when you registered and can be E-Signed.

    A Physical Evaluation is good for one year from the date of the doctor’s signature. A copy of that current physical must be uploaded into the Aktivate system during registration for a student-athlete to be eligible.

    What if I have a question during the registration process?

    Support will be provided by Aktivate, NOT Leon County Schools or Deerlake Middle School.

    How will I know my child has been cleared?

    After school administration has checked all documents and approved the forms submitted, your child will be cleared to participate in athletics on campus.

    Students are required to have a 2.0 GPA from the previous 9 weeks.  GPA requirements for 6th graders do not start until 2nd semester of 6th Grade. Please note that Athletic Physicals are valid for (1) year from the date of the doctor visit.
