2021SY Student Registration Information

  • Parents/Guardians:

    We welcome you to Swift Creek Middle School.  It is our goal to make registering your student easy as possible, but to do this we need your help.  

    To register a student you MUST be the parent or legal guardian, and live in the SCMS zone.  If you do not live in the SCMS zone you must be approved through Leon County School Choice. The School Choice office can be reached via their website at leonschoolchoice.com.  If you are unsure if you are in the SCMS Zone, please check the Leon County Schools website for the School Zone Lookup located under the Parents and Students Tab.

    Please review the following to determine enrollment requirements:

    Please read ALL the DETAILS carefully.

    Option 1: You are coming from a private school, out-of-state, or out-of-county, have never been enrolled in Leon County Schools, or have not been enrolled in Leon County Schools since 2015. (If you applied for school choice you may skip step 1)

    Step 1: You must pre-register your student online.  The link to register is:   https://leon.focusschoolsoftware.com/focus/auth/

    Step 2: Gather the following information:

    1. 2 proofs of residence:  A lease or mortgage, property tax record or homestead exemption AND a utility bill. If you are living with family or friends, please contact Guidance for additional paperwork.
    2. Birth Certificate
    3. Custody/Guardian Documentation (if applicable)
    4. ESE/504 paperwork (if applicable)
    5. Standardized test scores
    6. Immunization record – Must be on a Florida Form 680 – Check with your physician or the County Health Department
    7. Physical within last year
    8. Report card (from previous semester of school year) Must include all Middle School grades and promotion.
    9. Social Security card (not required)
    10. Withdrawal Form (from previous school)

    Step 3: Come to SCMS to register between 9:00 AM and 3:00 PM.  SUMMER HOURS may vary.

     

    Option 2: You are coming from a charter school or homeschool in Leon County

    Step 1: Gather the following information:

    1. If you are not in our school zone, and have applied for School Choice, please contact the School Choice Office (850)561-8484 if you have not received an approval letter to attend Swift Creek.
    2. Complete the LCS Student Registration Form. Please complete online and print.
    3. Complete and sign request for records form. This is not a fillable form so you will need to print and then complete.
    4. Once the forms are complete, bring them along with the following to the Guidance office to enroll:
      1. 2 proofs of residence:  A lease or mortgage, property tax record or homestead exemption AND a utility bill. If you are living with family or friends, please contact Guidance for additional paperwork.

    Step 1: Gather the following information:

Department Overview

  • The Guidance Office houses the guidance counselors, guidance secretary, registrar and school health assistant.

    The mission of the guidance department is to prepare students to become responsible, respectful, independent learners equipped with the critical thinking skills necessary to compete in our global society through academic, career and personal social guidance. Our guidance counselors offer brief solution based counseling. They work closely with our school resource officer, social worker, psychologist, staffing specialist, district attendance officer and district homeless liaison to ensure our students are provided with the necessary resources to be successful.

    The registrar’s office is available to assist with student records, the enrollment of new students and student withdrawals.

    Our school health assistant is available to receive medication from parents for student use (with the proper paperwork) and provide medical attention to injured and/or sick students.
