Registration Requirements

    Pre K and VPK students are registered through the LCS Pre K office. Please call 922-2099 for more information.


    The following documentation is required for enrollment    :
     
    Two Proofs of Residency
     
    Current Lease or Mortgage Statement. Also one of the following: 
    • Electric Bill/Commit for service receipt
    • Closing Statement/Final Settlement
    • Contract, Building permit and letter from builder stating the date of occupancy (with in the school year)
    • Construction loan and letter from builder stating the date of occupancy (with in the school year)
     
    Pre K students will also need a passport from the Pre K Office
     
     
    Residing with another family:
     
    • The same two required proofs of residency from the homeowner
    • Two documents mailed to that address in the parent/guardian's name
     
    Custody: 
    • Copy of the court order granting guardianship
    • Foster care: letter from DCF verifying placement
    • Any court order agreement(s) or restrictions pertaining to custody
     
     
     Also the following: 
    • Withdrawal form from prior school, current report card, transfer grades, copies of standardized tests
    • Birth Certificate
    • Immunization Record (on Florida Form 680D) or Exemption Certificate
    • Current physical (required for kindergarten, out of county/state transfers)
